Waiver and Release Statements for Camino del Espiritu on September 9-10, 2017
Liability Release and Consent: By participating in the Camino del Espiritu Pilgrimage (the “Pilgrimage”) on September 9-10, 2017, I hereby waive, release, and discharge any and all claims for damages for personal injury, property damages or which may hereafter occur to me as a result of participation in said event. This release is intended to discharge in advance The Archuleta Family Foundation, the Pueblo Diocese and its subsidiaries, their officials, officers, employees, volunteers and agents from liability, even though that liability may arise out of perceived negligence on the part of persons mentioned above. It is understood that some activities involve an element of risk or danger of accidents, and knowing those risks, I hereby assume those risks. It is further understood and agreed that this waiver, release and assumption of risk is to be binding on my heirs and assignees.
Medical Treatment: I hereby release and forever discharge The Archuleta Family Foundation and the Pueblo Diocese and its subsidiaries, their officials, officers, employees, volunteers from any claim whatsoever which arises or may hereafter arise on account of any first aid, treatment, or service rendered in connection with participating in the Camino del Espiritu Pilgrimage on September 9-10, 2017.
Photograph and Video Authorization: As a participant in the Pilgrimage, I hereby give permission to photograph or video tape me throughout the Pilgrimage and to use those photographs and videos for use in promotional materials, including but not limited to publications, website, and news releases, related thereto and without any financial compensation or rights of ownership stated or implied whatsoever.
